Testing and staging liver health
Most liver assessment no longer requires a biopsy. Modern practice uses a combination of blood-based scores and imaging to estimate how much fat and scarring is present — non-invasively, and in many cases in a single appointment.
The usual sequence:
Blood tests first. Liver function tests are often the starting point, though an important caveat applies — normal liver enzymes don't rule out liver disease. Significant fibrosis can exist with entirely normal LFTs, which is why scoring systems exist.
A risk score next. FIB-4 combines age, two liver enzymes and platelet count into a single number estimating advanced fibrosis risk. It uses results you may already have, costs nothing, and is widely used in Australian primary care to decide who needs further assessment. Its main limitation is a wide indeterminate range where the result is neither reassuring nor alarming.
Imaging where indicated. Elastography measures liver stiffness — a proxy for fibrosis — in a few minutes, painlessly. Some systems also measure liver fat. Different technologies exist, and the differences matter for both accuracy and cost: some are integrated with standard ultrasound, others are standalone, and this affects what Medicare covers.
