Type 2 diabetes risk check
Based on the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK). About two minutes — you'll need a waist measurement.
Educational estimate only. This form follows the themes of the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK) but is a simplified version. Confirm your result with the official AUSDRISK instrument and discuss it with your GP before making health decisions.
Build note: this must implement the AUSDRISK instrument accurately — the exact questions, scoring weights and risk bands as published by the Australian Government Department of Health. Do not approximate. Attribute the tool to its source, and verify against the current published version before launch.

Intro copy (above the tool):
This tool uses the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK), developed for the Australian population and used in general practice across the country.
It takes about two minutes and asks about age, sex, ethnicity, family history, waist measurement, physical activity, diet and a few aspects of your medical history. It doesn't require any blood tests.
Before you start: you'll need your waist measurement. Measure at the level of your navel, at the end of a normal breath out, with the tape snug but not compressing. Estimating from clothing size is the most common source of an inaccurate result.

Results copy — write one block per risk band, following the published AUSDRISK bands:
Low risk band:
Your score suggests a lower risk of developing type 2 diabetes in the next five years. This isn't a permanent result — risk changes with age, weight and activity, so it's worth rechecking periodically. The lifestyle factors below reduce risk regardless of your current score.
Intermediate risk band:
Your score suggests an intermediate risk of developing type 2 diabetes in the next five years. This is a useful point to act: the changes that reduce risk are most effective before glucose problems develop. Consider discussing this result with your GP, who may suggest a blood test.
High risk band:
Your score suggests a high risk of developing type 2 diabetes in the next five years, and it's worth taking to your GP. They'll likely suggest a blood test to check your current glucose — a high score doesn't mean you have diabetes, but it does mean checking is warranted. There's a genuine opportunity here. Type 2 diabetes usually develops through a reversible stage, and identifying it early is what makes intervention effective.

What this tool does and doesn't do
AUSDRISK estimates risk from factors known to predict type 2 diabetes. It does not diagnose anything — only a blood test can establish whether your glucose is currently raised.
A low score doesn't rule out diabetes, particularly if you have symptoms or a strong family history. If you're concerned, speak to your GP regardless of the result.
What reduces risk
- Losing a modest percentage of body weight if you're carrying excess, particularly around the middle
- Regular physical activity — both aerobic and resistance training
- Reducing refined carbohydrates, added sugars and ultra-processed foods
- Adequate sleep, and treatment for sleep apnoea if present
- Limiting alcohol
